Successful vaccine rollouts have failed to stop COVID transmission, with new data showing the prevalence of the virus increasing in fully jabbed individuals, according to a medical study in The Lancet.

Examining new infections in Germany, researchers found that the rate of cases among fully vaccinated individuals aged 60 and older has risen from 16.9% in July to 58.9% in October.

Offering a clear assessment that fully vaccinated people are increasingly becoming the source of COVID transmission, the study identified a similar situation in the UK. Throughout Britain, the number of household contacts exposed to unvaccinated cases (23%) was slightly lower than the number exposed to vaccinated individuals (25%).

In the space of three weeks in Britain, 100 COVID cases were reported among individuals who are 60 or older. Out of those infections, 89.7% of people were fully vaccinated, while 3.4% were unvaccinated.

More concerning for researchers, in Israel, an outbreak that infected multiple healthcare workers and patients, as well as their family members, came from a fully vaccinated hospital patient.

Backing up the fears expressed in the study, the US Centers for Disease Control and Prevention (CDC) currently lists four of the five areas across the US with the highest percentage of fully vaccinated individuals as high transmission counties.

Researchers warned that decision makers must acknowledge the risk that is still posed by vaccinated individuals.

The research released in The Lancet comes as countries weigh imposing COVID vaccine mandates to help protect citizens from the spread of the omicron variant and a potential resurgence in case numbers. Greece became the first nation in the EU to implement a COVID vaccine requirement, threatening all over-60s who don’t get the jab with a monthly fine.

AP — Ambulances filled with breathless patients lined up in Brazil as nations around the world set new records Thursday for COVID-19 deaths and new coronavirus infections. The disease surged even in some countries that have kept the virus in check.

In the United States, Detroit leaders began making a plan to knock on every door to persuade people to get vaccine shots.

Brazil this week became just the second country, after the US, to report a 24-hour tally of COVID-19 deaths that exceeded 4,000. India hit a peak of almost 127,000 new cases in 24 hours, and Iran set a new coronavirus infection record for the third straight day, reporting nearly 22,600 new cases.

In the state of Rio de Janeiro, emergency services are under their biggest strain since the pandemic began, with ambulances carrying patients of all ages to overcrowded hospitals struggling to care for everyone. Authorities say over 90% of the state’s intensive-care unit beds are taken by COVID-19 patients, and many cities are reporting people dying at home due to lack of available medical treatment.

“We’re already living the third wave. We have three times more calls,” in comparison with previous waves, said Adriano Pereira, director of the mobile emergency care service in Duque de Caxias, an impoverished city outside Rio.

Brazil’s death toll has risen past 340,000, the second-highest total in the world behind the US, where nearly 560,000 people have been confirmed killed. The US is the only other country that has had daily death tolls higher than 4,000. A Peruvian report of 4,143 virus deaths on Aug. 14 included deaths from several days.

Rio state’s 14-day moving average of COVID-19 daily deaths climbed from 112 to 207 between March 1 and April 7, with some health analysts expecting even worse days in the next couple of weeks. Many hospitals warn about the risk of shortages of oxygen and sedatives for intubation.

Indian Prime Minister Narendra Modi urged people to get vaccinated, writing in a tweet: “Vaccination is among the few ways we have to defeat the virus. If you are eligible for the vaccine, get your shot soon.”

The US has now fully vaccinated nearly 20% of its adult population, and New Mexico became the first state to get shots in the arms of 25% of its residents — milestones that are still far off for many hard-hit countries.

In India, home to 1.4 billion people, only 11 million are fully vaccinated. In Brazil, less than 3% of the country’s 210 million people have received both doses, according to Our World in Data, an online research site.

South Korea reported 700 more cases, the highest daily jump since Jan. 5. Health authorities were expected to announce measures to strengthen social distancing following a meeting Friday.

In Thailand, which has reported only 95 deaths during the pandemic, health officials reported the country’s first local cases of the coronavirus variant first detected in Britain. The news comes at a time when only 1% of the population has been vaccinated and as Thais prepare to celebrate the traditional Songkran New Year’s holiday next week, typically a time of widespread travel.

That variant is more contagious, and the US Centers for Disease Control and Prevention said this week that it is now the most common variant in the United States, raising concerns it will drive infections and cause more people to get sick.

Michigan has averaged more than 7,000 new cases a day — a number that makes the state second in the nation behind New York. Michigan also has the highest number of new cases per capita, with 1 of every 203 state residents getting diagnosed with COVID-19 between March 31 and April 7, according to data compiled by Johns Hopkins University.

In Detroit, which is about 80% Black, officials said they plan to start visiting homes to talk about the importance of protecting themselves from the virus with vaccinations and how to sign up to receive the shots.

“We’re going to knock on every residential door in the city, making sure every Detroiter knows how to make an appointment,” Victoria Kovari, an executive assistant to Detroit Mayor Mike Duggan, told The Detroit News.

Only 22% of Detroit residents have received at least one vaccine dose compared to 38% for all of Michigan, according to Michigan’s Department of Health and Human Services.
Other Midwestern states have seen troubling signs in recent days, including a school district in Iowa where 127 students and five staff members tested positive for the coronavirus or are presumed positive.

In Massachusetts, where the seven-day rolling average of daily new cases has risen to over 2,100 new cases per day, the Massachusetts Public Health Association called on Republican Gov. Charlie Baker to reinstate public health measures. The group urged Baker to limit indoor dining capacity and other indoor activities, saying the rise in cases and hospitalizations followed Baker’s decision to loosen those restrictions.

“We are currently in a race between the vaccines and the variants,” Carlene Pavlos, the group’s executive director said Thursday. “Without these public health measures, even more innocent lives will be needlessly lost.”

US President Donald Trump has called America’s painkiller-addiction crisis a “national shame” as he declared it a public health emergency.

Mr Trump announced a plan to target the abuse of opioids, which kill more than 140 Americans each day.

The president has previously promised to declare a national emergency, which would have triggered federal funding to help states combat the drug scourge.

The move instead redirects grant money to be used in dealing with the crisis.

Five ways to tackle the US drug epidemic

Mr Trump said on Thursday at the White House: “More people are dying from drug overdoses today than from gun homicides and motor vehicles combined.

“These overdoses are driven by a massive increase in addiction to prescription painkillers, heroin and other opioids.”

He added: “The United States is by far the largest consumer of these drugs using more opioid pills per person than any other country by far in the world.”

Mr Trump signed a presidential memorandum directing his acting health secretary to declare a nationwide public health emergency and ordering all federal agencies to take measures to reduce the number of opioid deaths, according to senior White House officials.

The order will also ease some regulations to allow states more latitude in how they use federal funds to tackle the problem.

But the White House plans to fund the effort through the Public Health Emergency Fund, which reportedly only contains $57,000 (£43,000).

The Trump administration will then work with Congress to approve additional funding in a year-end spending package, senior officials said.

Other elements of the directive include:

Allow patients further access to “telemedicine” so they can receive prescriptions without seeing a doctor

Make grants available to those who have had trouble finding work due to addiction

The Department of Health and Human Services will hire more people to address the crisis, particularly in rural areas

Allows states to shift federal funds from HIV treatments to opioids, since the two are linked as drug users often share infected needles

Proponents suggest Mr Trump’s announcement is a critical step in raising awareness about the nationwide epidemic, while some critics argue the move does not go far enough.

“The lack of resources is concerning to us since the opioid epidemic presenting lots of challenges for states’ budgets,” Michael Fraser, executive director of the Association of State and Territorial Health Officials, told Politico.

“My hope is people will realise with no new money the ball is going to be in Congress’s court,” he added.

Senator Bernie Sanders later tweeted that while Mr Trump was “right that the opioid crisis is a national emergency”, Friday’s announcement was “nothing more than an empty promise”.

“Millions depend on Medicaid [healthcare funding] for opioid treatment. Trump’s solution is to cut Medicaid by $1 trillion. That is a disgrace,” wrote the Vermont senator.

The 100 words that triggered the opioid crisis

Morgue services boom in US opioid crisis

Taking the first step

Rajini Vaidyanathan, BBC News, Washington

Addiction to painkillers and heroin has blighted so many communities across the US – both urban and rural.

As I travelled the country reporting on last year’s election, I remember the hairdresser in Arkansas whose ex-husband died from medicines he’d been given for his bad back, the family in New Hampshire who’d lost a teenage daughter to an overdose and heard stories of doctors who’d become hooked on the very pills they’d prescribed.

President Trump has stopped short of declaring this crisis a national emergency, despite earlier indications he would.

Instead his public health emergency is more of a short-term measure which doesn’t allocate as much funding. Recovering addicts and charities I’ve talked to say more investment in round-the-clock rehab and treatment is what is needed to make a difference.

But while today’s announcement is welcome, many will now be looking to Congress to take more action and secure more money to deal with this crisis.

Since 1999, the number of deaths involving opioids have quadrupled, reaching 33,000 deaths in 2015, according to the Presidential Commission on Combating Drug Addiction and the Opioid Crisis, citing data from the US Centers for Disease Control and Prevention (CDC).

The CDC first declared opioids, a class of pain medications as well as some street drugs, to be an “epidemic” in 2011.

Mr Trump first announced his intention to declare opioid abuse a “national emergency” in August.

“The opioid crisis is an emergency, and I’m saying officially right now: It is an emergency. It’s a national emergency,” he said at the time.

Experts had urged Mr Trump to use his presidential power under the Stafford Act to declare a national emergency, which would have given states access to money from the federal Disaster Relief Fund.

States would have had immediate access to funding, much like they would after a natural disaster.

But senior officials told reporters that declaring that sort of emergency was not a good fit for an ongoing crisis.

The announcement comes after Mr Trump’s pick for drug czar withdrew his nomination following a report that he helped neuter government attempts to tackle the opioid crisis.

Controversial Trump drug czar pick withdraws

Pennsylvania congressman Tom Marino pushed a bill that reportedly stripped a federal agency of the ability to freeze suspicious painkiller shipments.

Health Secretary Tom Price also resigned last month after it was revealed he was using expensive private planes for official business.

As a candidate, Mr Trump frequently pledged to tackle the drug crisis, and often campaigned in the hardest-hit states.
